Case Management
We provide non-clinical case management and peer recovery support to anyone in recovery or looking to reduce harms related to their substance use. Our team assists with common obstacles related to recovery such as homelessness, incarceration, legal issues, employment, education, transportation, healthcare, documentation and more.
Our team has a wonderful relationship with treatment facilities across the state which allows participants to gain access to treatment in the form of detox, rehab, intensive outpatient services, medication-assisted treatment, and more.
With both a case manager and peer support specialist on your team, we’re here to help you navigate your recovery journey based on our own lived experience. We’ll help you discover the recovery pathway that suits your needs, and help you to reach your self-defined goals.
