PLAINFIELD, NJ – May 27th, 2026 – Prevention Links announced today that it received a $5,000 grant from the Atlantic Health Overlook Medical Center Community Advisory Board, to support Harm Reduction Street Outreach in Plainfield, a program designed to engage residents living with substance use disorders—particularly those experiencing homelessness—through street outreach, harm reduction services, and connections to care.

These efforts are critical to reducing overdose risk and improving health outcomes in Plainfield.”

The Atlantic Health Overlook Medical Center Community Advisory Board, a group of individuals from throughout the communities served by the hospital, the hospital’s Community Health Advisory Committee and the Atlantic Health Overlook Medical Center Community Health Department, working with the Atlantic Health Planning Department, identified priority health needs within the community, using robust data analysis and a community stakeholder survey.


The Community Advisory Board awarded grants to 19 recipient organizations who proposed initiatives that address one or more priority health needs in this Spring 2026 period.


Through this initiative, Prevention Links will conduct weekly street outreach throughout Plainfield, led by Certified Peer Recovery Specialists. Outreach teams will engage individuals in public spaces, encampments, and other locations where people may be disconnected from traditional systems of care. Participants will receive essential supplies including water, snacks, wound care kits, safe sex kits, naloxone (Narcan), and other harm reduction materials. These resources meet immediate needs, prevent overdose and infection, and serve as a bridge to deeper engagement with recovery and support services.


The program will serve participants through direct outreach and engagement, focusing on individuals facing the greatest barriers to care and working to increase connections to treatment, reduce overdose risk, and improve overall health and stability.
